Understanding Prolotherapy

Prolotherapy (proliferative therapy) uses natural irritant solutions, typically dextrose (sugar water), to stimulate your body's healing response. It's often used for chronic ligament and tendon injuries, joint instability, and chronic pain conditions.

Treatment Series Approach

Unlike single-treatment procedures, prolotherapy typically involves a series of treatments spaced 1-2 weeks apart. Each session builds on the previous one, progressively strengthening the treated tissues. Most patients need 3 or more sessions for optimal results.

Before Your Treatment

Stop NSAIDs (Critical)

Stop anti-inflammatory medications 5-7 days before each treatment session:

  • Ibuprofen (Advil, Motrin)
  • Naproxen (Aleve)
  • Aspirin (ask your prescribing practitioner first)
  • Celebrex (celecoxib)

Why? NSAIDs block the inflammatory healing response that prolotherapy is designed to trigger.

What IS Okay

  • Tylenol (acetaminophen) — safe for pain relief
  • Most other medications — continue as prescribed
  • Eating normally — no fasting needed
  • Driving yourself — unless treating feet/ankles

Stay Hydrated

Drink plenty of water the day before and day of

Plan for 30-45 minutes for treatment

Wear Loose Clothing

Easy access to treatment area

After Your Treatment

First 24-48 Hours

  • Expect increased soreness — this is actually a good sign!
  • Avoid strenuous activity—light activity is fine
  • Use Tylenol only — still no NSAIDs
  • Heat may help — warm compresses can soothe soreness

Between Sessions

  • Continue avoiding NSAIDs throughout your treatment series if possible
  • Gradually return to normal activities
  • Note your progress — track pain levels and function
  • Sessions typically scheduled 1-2 weeks apart

The Healing Response

Prolotherapy works by triggering a controlled inflammatory response. Temporary increased pain and stiffness for 24-72 hours after treatment is normal and indicates the therapy is working. This discomfort typically subsides and improvement builds progressively with each session.
