Request AppointmentGather your relevant medical history to send in or bring with you. You will receive a link for self check-in before your visit where you can complete your medical history and intake forms. See What to Bring below for details.
What to BringDuring your initial consultation, Dr. Buchheit's team will review your history, examine you, and discuss whether you're a candidate for regenerative therapy.
What to ExpectPlease send the following items or bring physical copies to your appointment. Having this information ready helps Dr. Buchheit and his team thoroughly evaluate your condition.
Credit or debit card, check, or ACH payment accepted. Outstanding balances can also be paid through your patient portal.
Note: We are unable to accept cash payments.
Digital access to your most recent X-rays, MRIs, or CT scans related to your condition. If digital access is not readily available, a CD or physical copy of the imaging or radiology report will work.

Free parking is available in the parking garage adjacent to our building. Handicapped accessible spaces are located near the main entrance.
