Prepare for Your Appointment

Gather your relevant medical history to send in or bring with you. You will receive a link for self check-in before your visit where you can complete your medical history and intake forms. See What to Bring below for details.

Meet with Dr. Buchheit's Team

During your initial consultation, Dr. Buchheit's team will review your history, examine you, and discuss whether you're a candidate for regenerative therapy.

Please send the following items or bring physical copies to your appointment. Having this information ready helps Dr. Buchheit and his team thoroughly evaluate your condition.

Method of Payment

Credit or debit card, check, or ACH payment accepted. Outstanding balances can also be paid through your patient portal.

Note: We are unable to accept cash payments.

Imaging Studies

Digital access to your most recent X-rays, MRIs, or CT scans related to your condition. If digital access is not readily available, a CD or physical copy of the imaging or radiology report will work.

List of Current Medications

Include prescription drugs, over-the-counter medications, and supplements

Free parking is available in the parking garage adjacent to our building. Handicapped accessible spaces are located near the main entrance.

Building Access

Our office is located on the fourth floor. The building is fully accessible with automatic doors at the main entrance.
